Green WB shield center labels w/ correct release title and "After Forever (Including The Elegy)" as A2, incorrectly credited to "Ioomi-Ward-Osbourne-Butler". Song durations are shown as they appear on center labels, but are obviously incorrect.

Note that this release is similar to [r4272661], except that version's center labels incorrectly show "Masters Of Reality" as the title.

Catalog number "BS 2562" appears on back cover and center labels. "2562" appears on front cover only.
