Blue labels with silver logo and text. Labels have been crossed out with green marker (faded).

Catalog number is PS 476 on front & back cover and PS-476 on labels. "Paint It Black" is spelled with a comma on the front cover ("Including Paint It, Black") but without a comma on the back cover and on the label. "Going Home" is spelled with a "g" on the back cover, but spelled "Goin' Home" on the label.

The bottom of the labels read "Made In U.S.A." in silver text. There is no text to the left of the center hole (catalog number, side number, or stereo indication), only on the right side.

The bottom of the back cover reads: "PS 476 (also available in Mono, LL 3476) PLAY THIS RECORD ONLY ON STEREOPHONIC EQUIPMENT PS 476"
