Includes full-colour printed inner sleeves.
The liquid fill record comes separately, enclosed in an unsealed PVC sleeve.
Fold-out poster included, with the lyrics for each track printed on one side, and a photo on the other.

Limited to an unknown quantity.
Side D is etched, with the word "Mayhem". This side has no sound.
