Fourth issue features third sleeve variation (white text) with JC catalog prefix (ca. 1976-77), following the PC prefix of the first three issues (it would revert to PC a few years later).

Rear Pete Hamill sleeve notes from first sleeve [which was printed in black type], which were removed on the second sleeve, were reinstated on this, the third sleeve, by Columbia Records because Hamill's sleeve notes from the first sleeve had, by this time, won a Grammy award.
On this sleeve the typeface is white to make it more readable, and surrounds a small painting by David Oppenheim.

Hamill's actual sleeve notes were based on his hearing the test pressing, bearing the matrix numbers -1A. Dylan subsequently re-recorded some of the tracks with different (uncredited on the sleeve) musicians.

The first, second and third sleeves all house discs with a -2 set of matrixes.
