Front laminated square sleeve, with the octagonal cover reproduced on black background and perforated at the back to reveal the blue colour code of the poly-lined inner sleeve.

First cat.nr. on cover, second on label.

This version is very close to [r=1906094] but has a slightly different label layout & the side B cat# omits the full stop that is present on the side A cat# (ie: side A is SKL.5019 but side B is SKL 5019).
