This version has a date etched in Side D(or Side 4) of "9-5-68".

Side 2 has a locked groove on track B2-V during the drum fill of "Parthenogenesis" (approximately 2/3 of the way through the side). Another All Disc / Audio Matrix version (with same label layout and text design) exists without the B2 'lock groove' on [r=3025513], and has a later date of 9-25-68 etched in the runouts.

Inner sleeves are Liberty black on white print.
